My Aged Care

My Aged Care (MAC) provides support to older people who are over 65 years old. Occupational Therapists play a key role in addressing the physical, emotional, cognitive and environmental factors that impact older people’s participation in their daily activities.

Our Occupational Therapist delivers high quality, specialised services to older people with age-related conditions including dementia, poor balance/coordination, chronic pain, etc.

Our Occupational Therapist has extensive experience in providing the following services:

·       Home safety assessment

·       Falls prevention

·       Pressure care management

·       Prescribing a range of Assistive Technology (equipment) to improve participations and functions

·       Modifying the home to facilitate better access and participation in activities of daily living

·       Palliative care

Funding Systems

  • Home Care Package (HCP)

    The Home Care Package (HCP) provides individualised support services to older people who require ongoing high intensity care.

    Our Occupational Therapist works closely with Home Care Providers to provide Occupational Therapy services to support older people to allow them to safely remain living at home for as long as possible.

  • Commonwealth Health Support Programme (CHSP)

    The Commonwealth Health Support Programme (CHSP) provides entry-level support to older people to remain living at home and in their community. CHSP services are subsidised by the Australian Government. They may be short-term, intermittent or ongoing.

    Based on the clients’ needs, our Occupational Therapist helps older people to stay independent and safe in their own homes.

  • Short-term Restorative Care (STRC)

    The Short-Term Restorative Care is an early intervention programme that provides a range of goal-oriented and multi-disciplinary services to older people for up to 8 weeks in a community setting.

    If there is a requirement for Occupational Therapy services, we work closely with service providers to provide timely assessment and interventions. We also support older people to transition between care settings or funding systems.
